Primary Care Services Your Team in the San Fernando Valley

Our unique, team-based approach to primary care services combined with our urgent care services can give patients an easier and more enjoyable alternative to traditional primary care. When you’re a primary care patient at Mend and you make Mend your medical “homebase”, you no longer have to wait for an appointment for any urgent issue—simply walk-in to Mend for treatment at your doctor’s office, seven days a week. All urgent issues, medical information, referrals, coordination, and follow-ups become managed under one roof.

For preventive, screening and wellness services, schedule an appointment with Mend’s team of providers to go over your medical history and review your health goals. For acute and urgent issues, walk right in for treatment and let us coordinate your care throughout the healthcare system —no appointment is necessary.

As part of Mend’s primary care practice, you no longer have to decide between visiting your primary physician or other walk-in clinics for your medical issues. Your care is coordinated by Mend and your medical home becomes any one of our convenient locations in Sherman Oaks, Van Nuys, or Burbank.

What is Primary Care?

Primary Care refers to health promotion, disease prevention, health maintenance, counseling, patient education, and on-going treatment of acute and chronic illnesses. Primary Care is overseen by an assigned Primary Care Physician (PCP) and performed by a team of medical professionals, often collaborating with other health professionals outside of the practice and utilizing consultation or referrals as appropriate.

By contrast, Urgent Care refers to the immediate treatment of acute injuries and illness–such as coughs, colds, and flu-like symptoms–that are not life-threatening but should be treated within 24 hours. Urgent medical issues can be treated by a traditional Primary Care office, an Urgent Care center, or an Emergency Room.

What visits are considered Primary Care vs. Urgent Care?

Primary Care visits are designed to offer counseling, health education, and health maintenance for on-going medical issues and to monitor your health against lifestyle goals over time. Visits considered Primary Care include:

  • Annual Wellness Visits
  • Complete History & Physical Examinations
  • Routine Lab Testing & Screenings
  • Vaccinations

Primary Care visits are by appointment-only.

By contrast, Urgent Care visits do not require an appointment and can address on a walk-in basis urgent, acute, and immediate medical conditions that are not life-threatening.

What is required to schedule a Primary Care appointment?

To prepare for your initial Primary Care visit, we ask that bring any previous medical records, a list of health and lifestyle goals, a list/bottles of any medications you are taking, and your insurance card.

Please complete this Medical Records Authorization Form and provide to your previous doctor’s office or to Mend prior to your first appointment to allow for acquisition of your medical records and medical history. Completed forms can be e-mailed to vannuys@mendurgentcare.com or faxed to 818-646-2697.

Primary Care visits thereafter that qualify as either a routine wellness visit, physical, or routine lab testing/screening/vaccination will require an appointment. Urgent Care visits do not require an appointment.

Who will I see for my Primary Care visit?

Your care will be overseen by your assigned Primary Care Physician (PCP) and will be performed by our team of high-quality medical professionals in addition to your PCP to ensure timely, quality, and available care that is convenient to you and your schedule.
